How do we support pupils changing classes or schools?
We work closely with all schools and personnel to make transition as smooth as possible. Steps that are taken (as appropriate) to support the transfer include:
- Extra visits for the pupils with familiar adults tailored to suit the individuals
- Involvement of a Social Inclusion Worker
- SENCo attends the Final Review meeting for pupils with a Statement or EHCP in the term prior to transfer to Westfield
- Secondary SENCos are invited to attend the Year 5 transfer review and the final review meeting for pupils with an EHCP prior to transfer from Westfield, and any other appropriate meetings
- Year 6 classteachers and SENCo meet with St Ivo Secondary SENCo or SENCo Support to share information/records
- A member of staff from the Secondary School may visit Westfield and meet with the child prior to transfer
- Parents/Carers can make an appointment to meet the SENCo and discuss their child and the School’s provision
- Relevant information and records are shared and passed on
- Extra focused learning about aspects of transition is put in place for those who are particularly anxious or who need more support
- Within school Class teachers meet to discuss pupils and to share Pupil Interview and Intervention sheets, prior to transfer to the next year group
- Pastoral support information is shared to ensure continuation of useful strategies in the form of the Pupil Interview sheets/passport
